COVID-19 Vaccine Support Services Include:

• Consumer education about the importance of receiving a vaccine and booster. 
• Identifying individuals unable to independently travel to a vaccination site. 
• Center staff assisting with scheduling a vaccine appointment. 
• Center staff arranging or providing accessible transportation.
• Providing companion/personal support during the process.
• Reminding individuals of the second vaccination appointment and booster as needed.
• Center staff providing technical assistance to local health departments or other entities on vaccine accessibility.

Center staff will be reviewing service requests and responding to the needs of individuals with disabilities.  Centers are authorized to incur a limited range of operating expenses directly related to the COVID-19 Vaccine Services.   If you are a person with a disability or know someone with a disability and reside in Passaic or Essex Counties please complete and submit the brief form on our home page.
